How do I know what pharmacy is assigned to my file?

To know what is the primary pharmacy that we have assigned to your chart (i.e. the default pharmacy where your prescriptions are sent) please do the following:

• Login to your patient portal
• Click on my health in the side menu
• Click on medications
• Scroll down and look under the section that says pharmacy and you will see the names of the pharmacies that we have assigned to your profile
